A special part of attending Identity Church is the opportunity to serve in one of our many departments on Sunday mornings and events hosted by our church family. Please respond to this questionnaire regarding the various serving departments by selecting your top 3 choices. We would love to know where you desire to serve, have experience serving, as well as any training you have that may be helpful while serving. Whatever team you are on, you will be a valuable asset.

Waivers & Releases

Liability Waiver *

On behalf of me and my household, we understand that we assume all risks and hazards related to incidental participation, including attendance, games, outreach, and activities related to Identity Church. We waive, the release and absolve any indemnity and agree to hold harmless any and all volunteers of Identity Church and associated departments, volunteers, staff and leadership, for any claim arising from any injury (or previous injury) or damage to our child or his property, or any other reason. In the event of an emergency I grant Identity Church permission to transfer me to a safe location and if needed call 911 to seek medical help. Release of information: I hereby agree that anything shared with Identity Church and/or any elder, staff or director, that relates to my mental, spiritual, physical, or emotional health and well-being, or the mental, spiritual, physical, or emotional health and well-being of others, including past crimes or wrongdoings I may have been committed or that I have knowledge of, may be shared with law enforcement and/or applicable health professionals or government agencies.

Medical Emergencies *

In the event of a medical emergency, I understand that Identity Church’s policy is to immediately call 911. I grant permission to Identity Church staff, volunteers, and/or affiliates to transfer me and/or my child(ren) to a safe location. Then if applicable for emergencies amongst those 18yr or younger notify the parent/guardian. I grant Identity Church staff, volunteers or affiliates to perform life saving measures such as CPR or the Heimlich maneuver in the event of an emergency or a situation that appears to be an emergency. I agree to hold Identity Church, its staff, volunteers, and affiliates harmless for any injuries that could result from these life saving measures.

Photo & Media Release *

On behalf of me and my household, we understand that by checking this box I am granting permission for Identity Church to photograph my child participating in activities, etc. I understand that this consent gives Identity Church permission to publish these pictures, videos, and audio content. I agree that Identity Church may photograph and record my child/dependent’s likeness and activities (Images) during church-related activities. I grant the following rights to Identity Church: permission to use and re-use, publish and re-publish, and modify or alter the Image(s) taken during the shoot. Use of the Images for editorial, commercial, trade, advertising, and any other purpose may be done in any medium now existing or subsequently developed, on the church website and on the Internet, and worldwide in perpetuity for the purposes stated above. I waive my right to inspect or approve any editorial text or copy that is used in connection with the Images and release and discharge Identity Church from any and all claims arising out of use of the Images for the purposes described above, including any claims for libel, invasion of privacy, or other tortuous act. I have read the foregoing. I fully understand its contents, understand that this agreement does not expire, and confirm my agreement by checking this box.

Identity Church Honor Code *

This honor code is in place to provide a standard to anyone who desires to serve at Identity Church. *By checking this box, you also verify that you agree to the statement of beliefs set forth by Identity Church and commit to abiding by those beliefs in thought and action. As followers of Jesus, we believe it is our commitment to the Lord to live a holy and blameless life before God. Living above reproach is a way of life measured by the heart and dedication of each person serving in the Identity Church family. We should regard it as an essential part of our growth in the Lord, not as an imposition or restriction. Our desire is that those who serve at Identity Church maintain a disciplined life of daily time with the Lord in prayer and reading of the Bible and that you are faithful in regular attendance to Identity services. We ask all those serving to refrain from things such as: Ungodly Speech - Negative behavior such as gossip, slander, and profanity as we seek to honor the Lord in our words (This pertains to posting on social media as well.) Addictions - Gambling, illegal or recreational drugs, and over-indulgence in alcoholic beverages (drunkenness). Sexual Immorality - Living together outside of the Biblical view of marriage. Engaging in sexual activity outside the Biblical mandate of sexual activity within the marriage covenant between a man and woman, including watching pornography. We also ask that all those serving read through our statement of beliefs on our website and adhere to those beliefs. *By checking the box above, you also verify that you agree to the statement of beliefs set forth by Identity Church and commit to abiding by those beliefs in thought and action. You also agree and understand that you may be asked to step down from serving if this honor code is not adhered to.
